every half an hour means every tick. if you train at 14:23, it doesn't mean that 1 hour later (15:23) the unit will be healed by 60HPs, but that at 14:00. at 15:30, it'll be healed for 90HPs.

take a random battle, where you have a unit hp of 60. it gets injured by 35, so has hp 25.

how long does it take to heal when a) training at :15
b) training at :29


in both cases, it takes 2 ticks to get healed. so at :30 the unit'll be injured for 1 tick and at :00 it'll be fully healed. the hospital only shows how many ticks the unit is injured, not how many HPs it has yet to heal. if you trained at :01 or :31, you will most likely have to wait 1 tick less.

The effect is the same. A unit injured later in a tick will have a bigger chance of being injured for an extra tick. The cause is irrelevant; you should just train at the start of a tick.
